Purple Book appointments

If you wish to CANCEL or RESCHEDULE an appointment, PLEASE CALL 1300 749 869 (8.30am to 4pm weekdays excluding public holidays).

For all other queries, (including making non-purple book appointments) please contact your Child Health Centre.

If you proceed with this registration, you will be asked to provide personal information such as your child’s name and your telephone number. This information will be used by the Department of Health for booking appointments only.
